EQUIPMENT RENTAL LIABILITY WAIVER
This Liability Waiver (“Waiver”) is entered into on this [Date] between the undersigned individual (the “Participant”) and Tribe Academy LLC (dba Combat Rentals) (“Company”), collectively referred to as the “Parties.”

RELEASE OF LIABILITY: In consideration for being permitted to use the rented equipment provided by the Company, the Participant agrees to the following:
ASSUMPTION OF RISK: The Participant acknowledges that the use of the rented equipment involves inherent risks, including but not limited to the risk of injury, property damage, and other unforeseen risks. The Participant voluntarily assumes all such risks.
WAIVER AND RELEASE: The Participant hereby releases, discharges, and holds harmless the Company, its officers, employees, agents, and representatives from any and all claims, liabilities, demands, actions, or causes of action, whether at law or in equity, arising out of or in connection with the use of the rented equipment.
INDEMNIFICATION: The Participant agrees to indemnify and hold the Company harmless from any and all losses, liabilities, damages, or costs (including attorney fees) incurred as a result of the Participant’s use of the rented equipment.
CONSENT FOR MEDICAL TREATMENT: In the event of an injury or illness during the use of the rented equipment, the Participant authorizes the Company to secure necessary medical treatment for the Participant.
AGREEMENT TO COMPLY WITH RULES: The Participant agrees to comply with all rules and guidelines provided by the Company regarding the use of the rented equipment.
GOVERNING LAW: This Waiver shall be governed by and construed in accordance with the laws of the State of Texas.
ENTIRE AGREEMENT: This Waiver constitutes the entire agreement between the Parties and supersedes all prior discussions and agreements, whether oral or written.
ACKNOWLEDGEMENT: By signing below, the Participant acknowledges that they have read and understood this Liability Waiver and voluntarily agree to its terms.
