COMBAT RENTAL EQUIPMENT LIABILITY WAIVER
This Liability Waiver (“Waiver”) is entered into on this [Date] between the undersigned individual (the “Participant”) or, if the Participant is under the age of 17, the undersigned parent or legal guardian (the “Parent/Guardian”) and Tribe Academy LLC dba Combat Rentals (“Company”), collectively referred to as the “Parties.”

RELEASE OF LIABILITY: In consideration for being permitted to use the equipment provided by the Company, the Participant (and Parent/Guardian, if applicable) agrees to the following:
- ASSUMPTION OF RISK: The Participant acknowledges that the use of equipment involves inherent risks, including but not limited to the risk of injury, property damage, and even death. The Participant voluntarily assumes all such risks.
- WAIVER AND RELEASE: The Participant (and Parent/Guardian, if applicable) hereby releases, discharges, and holds harmless the Company, its officers, employees, agents, and representatives from any and all claims, liabilities, demands, actions, or causes of action, whether at law or in equity, arising out of or in connection with the use of the equipment.
- INDEMNIFICATION: The Participant (and Parent/Guardian, if applicable) agrees to indemnify and hold the Company harmless from any and all losses, liabilities, damages, or costs (including attorney fees) incurred as a result of the Participant’s use of the equipment.
CONSENT FOR MEDICAL TREATMENT: In the event of an injury or illness during the use of the equipment, the Participant (and Parent/Guardian, if applicable) authorizes the Company to secure necessary medical treatment for the Participant.
AGREEMENT TO COMPLY WITH RULES: The Participant agrees to comply with all rules and guidelines provided by the Company regarding the use of equipment.
GOVERNING LAW: This Waiver shall be governed by and construed in accordance with the laws of the State of Texas.
ENTIRE AGREEMENT: This Waiver constitutes the entire agreement between the Parties and supersedes all prior discussions and agreements, whether oral or written.
ACKNOWLEDGEMENT: By signing below, the Participant (and Parent/Guardian, if applicable) acknowledges that they have read and understood this Liability Waiver and voluntarily agree to its terms.
